THE ROGUE VALLEY UROLOGY BEND OFFICE IS PERMANENTLY CLOSED. 

CASCADE LAKES UROLOGY WILL RE-OPEN IN IT’S PLACE AS OF JULY 1, 2025.

The Rogue Valley Urology Bend office has become Cascade Lakes Urology.  This transition reflects our continued commitment to providing you with the highest quality urologic care in a patient-centered environment.

As part of this change, Dr. Andrew Neeb, Dr. Marta Johnson-Mitchell, and Suzanne La Lande, PA-C, will be leaving Rogue Valley Urology to form Cascade Lakes Urology.  While our name is changing, our mission remains the same – to offer you the same expert care, comprehensive services, and compassionate treatment that you have always received from us.

What’s Staying The Same?

  • Our address will not change – you will still visit us at our current location for all your urology care needs.
  • The same trusted providers and services – you will continue receiving the same high-quality care from the same team.

What’s Changing?

  • New name – we will officially become Cascade Lakes Urology on July 1, 2025.
  • New Electronic Health Records (EHR) system – we are upgrading to a new EHR system to enhance your experience and streamline your care.  As part of this transition, you may need to register for a new patient portal to access your medical records, request prescriptions, and communicate with your provider.  We will provide instructions and support to make this as easy as possible.

Medford Office

THE MEDFORD OFFICE IS PERMANENTLY CLOSED.

Dr. Bryan has left the area.  Dr. Bui, Dr. Davol and Michael Trinkler, PA-C have joined Providence Medical Group (PMG) Medford Urology Clinic.  If you would like to be seen at the new PMG Medford Urology Clinic, please have your primary care provider send a referral.  Even though you may have seen one of these providers in that past, you will need a referral to establish care at the PMG Medford Urology Clinic.  You will need to obtain a copy of your medical record by contacting Desert River Solutions if you were seen at Rogue Valley Urology, as Providence will not have access to your records.  PMG Medford Urology can be reached at 541-204-1699 or by visiting:  https://www.providence.org/locations/or/professional-plaza-medford/urology.

MEDICAL RECORDS
