ROGUE VALLEY UROLOGY HAS CLOSED BOTH ITS MEDFORD AND BEND OFFICE LOCATIONS PERMANENTLY.

Cascade Lakes Urology will open in the Bend office location as of July 1, 2025.

Providence Medical Group Medford Urology has opened in the Medford office location.

Patient records will be available by faxing in a Release of Information Form. 

Bend patients fax a Release of Information Form to 541-508-7968. 

Medford patients fax a Release of Information Form to 541-732-3910.
